Hiring Range: $3,976 - $5,302 per month
Summary of Work
This position will be primarily assigned to Youth Transitional Services and provide social work duties to individuals with intellectual disabilities and their families who reside at the Life Skills & Transition Center. These services include but are not limited to; family relationship building, assessments, counseling and therapy, admission and discharge planning, programming support and team participation. The YTS Social Worker is responsible for YTS admission-transition-discharge along with the additional responsibilities with school-day coordination and primary family coordination. This individual will work with other disciplines providing support for staff training opportunities, generally including up to 2 weekend days and 3-4 evenings per month. If and when decrease in census your caseload could transition over to adult services. With each person's Program Coordinator, this person oversees the development and implementation of Person Centered Transition plans for all people living at the LSTC. Highly involved in the Clinical Assistance Resources and Evaluation Services (CARES) both onsite and in our future statewide services. Back-up to the Social Services Director.
Minimum Qualifications
Requires licensure as a Licensed Social Worker (LSW) by the North Dakota Board of Social Work Examiners (NDCC 43-41); and one year of professional human services work experience as a social worker, human relations counselor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector; OR a master's degree in social work and licensure as a Licensed Social Worker (LSW) by the ND Board of Social Work Examiners.
If unable to find qualified applicants, will consider filling the position as Social Worker I, which requires licensure as a Licensed Social Worker (LSW) by the North Dakota Board of Social Work Examiners (NDCC 43-41). Salary range $3657-$4877 per month. Oversees the development of and implementation of Person Centered Transition Plans for all people living at the LSTC. Highly involved in the LSTC CARES - both on-site and in our furture statewide services. Back up to the Social Services Director.
